The Pennsylvania Donate Life Donor Registry is a compiled database of persons 18 years of age or older who made the decision to be an organ donor. The registry allows professionals in the organization to confirm at the time of death if you registered to be a donor and that your choice to donate can be fulfilled.
It only takes 90 seconds or less to add the organ donor designation to a driver's license or identification card by visiting dmv.state.pa.us and selecting the Online Services link, where the option to add the organ donor designation is found under Driver's License/Photo ID Services. Once the designation is added, individuals will receive a designation card that they carry with them to affirm organ donor status until they renew their driver's license or identification card.
Go to dmv.state.pa.us then access the form by selecting the Online Services link.
