Changing your name on a Pennsylvania Drivers License

  • For a commercial drivers license name change, a person must fill out form DL-80CD. However, if the person's drivers license expires in the next six months, they must fill out a DL-143CD to renew their drivers license with the name change.
  • To change your name on a learners permit, you must fill out form DL-31.
  • For a non-commercial drivers license name change, you must fill out form DL-80.  If your drivers license expires within the next six months, you must fill out form DL-143 to go ahead and renew their drivers license with the drivers license name change.
  • For a photo identification card, you must simply fill out a DL-54B form.
